I just do two friendly interactions and then ask for the skull. It worked every time regardless of the relationship between the two sims.

Gravestones are available in debug and you can get several skulls by just asking celebrating sims. It’s a free challenge for people who want to participate and easily avoidable for those who wish not to partake.
