As for illness, the most I get is a moodlet that goes away by itself. No patient has ever died on me on GTW. In fact, I have to game my playstyle a lot to get anything but delirious happy Sims.
I’ve played every generation of TS so far and TS4 is by far the least challenging, in that I agree with Anne-tione.
